Some prescription-based services require the Functional Health membership for clients needing ongoing prescription management.
Monthly Membership Plans
Choose the level of ongoing care that fits you — from wellness guidance to full functional and integrative healthcare. All memberships require a 6-month minimum commitment and a card on file, billed monthly.
Holistic Health
Ongoing wellness guidance & provider check-ins

Functional Health

- Preferred lab pricing; 10% off supplements & IV therapy

Concierge Health
The highest level of access & health optimization
- 6-month minimum commitment
- Priority scheduling & messaging (business hours)
- Same-day / next-business-day visits when available
- Functional lab review & personalized optimization
- Prescription management included
- Enhanced care coordination & health coaching
- 15% off eligible supplements & IV therapy
Additional NP visits beyond those included: $125 for up to 60 minutes, $75 for up to 30 minutes. Additional health-coaching sessions (Holistic Health): $50 each.
Prescription management requires either the Functional Health membership ($149/mo) or a Concierge Health membership; it is not included in Holistic Health. Clients who need ongoing or recurring prescription medications must maintain the Functional Health or Concierge Health membership — pay-as-you-go, à la carte pricing is not available for recurring prescription management. A membership does not guarantee that any medication will be prescribed, continued, increased, decreased, or refilled — all prescribing, refill, and monitoring decisions are made by the provider based on clinical appropriateness and applicable law. Some medications require an in-person or telehealth evaluation and/or lab monitoring, with follow-up required at least every 3 months for certain medications. Medication costs are billed separately from membership fees.
Laboratory testing is billed separately from the membership fee unless stated otherwise. Functional Health members using insurance may be given conventional lab orders where clinically appropriate so they can use their insurance benefits.
Concierge Health does not provide 24/7 emergency medical care or guarantee immediate access to the provider. Medical emergencies should always be directed to the appropriate emergency medical service or emergency department.
